A 66-year-old female patient with a history of rectal carcinoma received chemo-irradiation followed by surgical resection, presented by elevated CEA. Her PET/CT assessment shows a axial CT, b axial PET, d sagittal PET, c axial PET/CT, f sagittal PET/CT PET CT showing the following: heterogeneous hypermetabolic soft tissue nodule at the operative bed (along the right basal aspect of mesorectal fascia) measuring 19 × 10 mm achieving 19.8 SUVmax (a, b, f); another smaller intensely avid one measuring 14 × 10 mm is seen opposite the tip of coccyx achieving 16.8 SUVmax (f); and bilateral enlarged hypermetabolic inguinal lymph nodes achieving 23 and 13 SUVmax (c)
