A 38-year-old female with Budd-Chiari and occluded index TIPS (previous 3 telescoping stents). Initial portal puncture was done at portal neck (arrow) and portogram showed occluded index TIPS (a). Balloon dilatation of the tissue between portal and cava (b). Portography before insertion of PS delineates the PV branches (c). Final portogram after PS creation showed good flow through the PS with shunting of blood to cava. CT thin-MIP showed the PS and index stent and the later seen longer inside the PV than the PS (e)
