A 72-year-old female patient presented with severe dyspnea with desaturation. She was intubated. CT chest revealed bilateral symmetric confluent patches of ground glass with crazy paving appearance mixed with consolidative patches with air bronchogram more in the lower lobes (white arrows) more on the right side. LT side mild pneumothorax was noted (black arrows). It was diagnosed as AIP after transbronchial biopsy
