A 48-year-old patient had undergone right modified radical mastectomy for breast cancer and came to us for screening of the left breast after 1 year. One post-operative scan after 6 months of operation had been done in a private diagnostic center which was reported as normal. In the current scan, 6 months after the first postoperatives scan, a focal area of architectural distortion (notched tail arrow) was visible on DM (A) and DBT (B) images. USG (C) also revealed a focal heterogenous area with posterior shadowing and BIRADS 4 assessment was made. US guided biopsy was done and HPE was invasive carcinoma
