A twenty seven years old male patient, diagnosed with papillary differentiated cancer thyroid. underwent total thyroidectomy, followed by 120 mCi RAI-131 ablation. Follow up negative iodine whole body scan done 1 year later, inspite of elevated thyroglobulin level (= 12 mg/dl). PET/CT was done and showed right retro-mandibular/pre-auricular soft tissue nodule (nodal) with SUVmax ~ 7.2.
