An 48-year-old female patient, presented with breast cancer, and underwent fine-needle aspiration cytology from the most two suspicious lymph nodes, Tru-cut needle biopsy didn’t obtain in this case as suspicious lymph nodes were multiple and small in size, The final cytological evaluation showed metastatic lymph nodes harboring malignancy. a, b Axillary ultrasound with Doppler shows multiple suspicious lymph nodes, the largest was oval in shape and measures about 21 × 12 mm, with effaced hilum. The other nodes are round in shape the and largest of them measuring about 10 × 9 mm. On color Doppler, the lymph nodes showed both hilar vascular flow in addition to some cortical non-hilar blood flow at the periphery. c, d show FNA needle arrows while traversing the cortex of both lymph nodes, in which histopathological analysis revealed metastatic malignant cells of both aspirated nodes. e Photomicrograph of the largest lymph node cytology smear: clusters of malignant epithelial cells on hemorrhagic (Papanicolaou stain, 400X)
