A 57-year-old male bilateral dull aching orbital pain, epiphora and cervical lymphadenopathy. Axial T1 image A shows T1 hypointense infiltrative masses involving both lacrimal glands. Coronal T2 image B revealed hyperintense signal of the lesions. Sagittal T1 post-contrast C reveals homogeneous mild enhancement of the previously mentioned both lacrimal gland masses. (arrow) Coronal T1 post-contrast image D shows mild enhancement of the lacrimal gland lesions. Evident diffusion restriction on DWI (b1000)/ADC (E, F).
