A 8-month-old female baby, known case of Down syndrome and congenital heart disease (VSD, ASD, and PDA), presented by acute dyspnea with severe respiratory manifestation. CT shows bilateral upper and lower lobar posterior consolidation and atelectasis (note chest tube is seen in the right lung due to pneumothorax (resolved)), picture atypical for COVID, and CT severity score was 15/20.
