A 52-year-old male patient, known to have Crohn's disease for 15 years on Remicade and Imuran, presented with discharging fistula at the site of ileal resection–anastomosis for follow-up. Axial and coronal T2-w SPAIR images A, C show circumferential mural thickening affecting the distal ileum, with complicating branching fistulous tract, fat smudging and right paraspinal and right psoas inflammation, and impending abscess formation (arrows) and axial T2-w SPAIR (B) image showing fluid level within a skipped area which is noted at caecum (arrow).
