A 45-year-old male patient, known with Crohn's disease for 2 years on mesalazine (Pentasa) and steroids, presented with persistent diarrhea for follow-up. Axial and coronal post-contrast THRIVE images A, B show circumferential mural thickening and trilaminar pattern of enhancement at the distal ileal loop, axial T2-w SPAIR image C shows a short fistulous tract connected to the aforementioned loop (arrow) and coronal post-contrast THRIVE image D shows vascular congestion at the mesenteric side of an ileal loop (Comb sign). Ileo-colonoscopy E showed hyperemic ileal mucosa with patchy linear scarring
