A–D images: A 38-year-old female patient complained of left hand pain, tingling, numbness mainly at the thumb, index, middle finger, and radial half of the ring finger, and weakness of thumb abduction with history of wrist trauma 4 months ago. By nerve conduction studies, the case was diagnosed as severe carpal tunnel syndrome. A, B Axial T2SPAIR images showing increased signal intensity and flattening of the examined median nerve (green arrow) as well as bowing of the flexor retinaculum (white arrow), C ADC map showing low mean ADC value of MN at DRUJ (0.92 × 10–3 mm2/s), and D ADC map showing increase of mean ADC value at pisiform (1.32 × 10–3 mm2/s).
