Shown is a CT coronary angiogram of a 60-year-old male patient chronic heavy smoker, dyslipidemic, and hypertensive. For the previous few months, he experienced recurrent attacks of retro-sternal chest pain that were precipitated by effort and relieved by rest. Resting ECG had no ischemic changes. HeartScore = 24 (very high risk) and CACS = 215.MSCT-CA images show significant distal left main and significant proximal LAD. RCA and LCX are free from significant disease
