In the left breast, there was an irregular mass of iso T1 SI (a), intermediate high T2 SI (b), high STIR (c) with strong restricted diffusion in DWI (d), low ADC value (e), and strong enhancement in post-contrast scan (f). It was associated with skin thickening and edema as well as suspicious axillary lymph node. Eccentric within the mass was a thin walled cyst of high T1 SI (a), high T2 SI (b), and bright in DWI (d), but for being suppressed in STIR sequence (c, arrowed), the probability of fatty versus hemorrhagic content was equivocal. In the post-contrast sequence (f), in which fat frequency-selective saturation technique was used, the cyst was not suppressed indicating it was hemorrhagic and not fatty. Also, an intracystic extension of the malignant lesion could not be defined within the hyperintense contents. The histopathology confirmed it was invasive duct carcinoma, engulfing a complicated cyst
