A 66-years old diabetic female patient presented with high grade fever and right loin pain. CT axial and coronal images demonstrate hydronephrotic changes with gas accumulated in the right pelvicalyceal system, it measures 8.5 × 5.6 × 13.7 as regard greatest axial and cranio-caudal dimensions with total air locules volume 327 cc. the patient was managed unsuccessfully by medical treatment, and drainage was done by ureteric stent
