A 25-year-old male presented by acute fever, headache, vomiting and neck stiffness was diagnosed as tuberculous meningitis. A axial pre-contrast FLAIR: shows minimal sulcal hyperintensity. B axial CE-FLAIR: depicts pronounced leptomeningeal enhancement around basal cisterns, with additional thin sheet of dural enhancement. C axial pre-contrast T1WI. D axial CE-T1WI: shows subtle sulcal & leptomeningeal enhancement around basal cisterns not appreciated as in B and could be easily confused with normal vascular enhancement
