Dilated form of infiltrative cardiomyopathy secondary to iron overload. Ten-year-old male with thalassemia major presented with signs of heart failure. Short axis and long axis in four- and two-chamber cine images (a, b, and c, respectively) showing marked dilation of both ventricles (LVEDV = 121.6 ml and RVEDV = 140 ml) with global hypokinesia and reduced LVEF (20%) and RVEF (39%). Short axis gradient-echo imaging sequence (d) show decreased myocardial signal more in infero-septal segment (circle in d). T2* WI (e) shows myocardial iron overload of heart together with decay curve in excel sheet (f). Both show heavy iron overload in heart and rapid signal loss with (T2* value = 8.8 ms)
