B-mode US (a) revealed a hypoechoic right lower deep cervical lymph node with irregular outline and lost hilum. L/S ratio was 1.9. Color Doppler (b) revealed a central pattern of internal vascularity. The split-screen US elastogram (c) revealed type 3 color pattern. Strain ratio value was 3.32. The final histopathological diagnosis was nasopharyngeal cancer
