A–E images: A 23-year-old female patient has presented with right hand pain, tingling, numbness mainly at the thumb, index and middle fingers, and radial half of the ring finger since 8 months. By nerve conduction studies, the case was diagnosed as severe carpal tunnel syndrome. A Axial T1, and B Axial T2-weighted MR images show manifest thickening of the examined median nerve with bowing of the flexor retinaculum (yellow arrows), C T2-SPAIR image shows increased signal intensity of the median nerve (yellow arrow), D ADC map showing low mean ADC value of MN at DRUJ (0.92 × 10–3 mm2/s), and E ADC map showing increase of mean ADC value at pisiform (1.3 × 10–3 mm2/s). These MRI features are consistent with carpal tunnel syndrome
