TB ankle joint- A 35-year-old female patient presented with pain and swelling around ankle joint since 3 months. Plain radiograph of ankle joint (A) shows joint space widening with lytic area in distal epiphysis and metaphysis of fibula (arrow) with adjacent soft tissue swelling. On MRI-Coronal PD FS (B) shows joint effusion at ankle and talocalcaneal joints. Sagittal T2 (C) and post-contrast axial (D, E) images show altered signal intensity area with post-contrast enhancement seen in distal metaphysis of fibula (oval). Thick enhancing synovium extending along tendon sheaths (arrow) with small peripherally enhancing collection (arrowhead) on lateral aspect of foot. Findings suggestive of infective aetiology likely tubercular arthritis. On follow-up, patient responded well to anti-tubercular treatment
